Open Gym / Birthday Party / Party Release-Waiver
This release must be signed and turned in by anyone participating in a party, birthday party or open gym at Cheer & Dance Express. Students will not be eligible to participate if they have not brought this form. If you are having a bithday party we suggest that you send this release out with your invitations in order to get them back in time.
